Chrissy Teigen has unveiled a touching tribute to her late son, Jack, who tragically died after the model suffered a miscarriage 20 weeks into her pregnancy.

The 34-year-old model and presenter revealed the heartbreaking news of her miscarriage back on October 1, sharing a series of raw pictures from the hospital.

Teigen revealed in her candid and honest post that "despite bags and bags of blood transfusions", her unborn son did not survive.

In the emotional post, Teigen also revealed that she and husband John Legend had decided to name their unborn son Jack:

"We never decide on our babies’ names until the last possible moment after they’re born, just before we leave the hospital. But we, for some reason, had started to call this little guy in my belly Jack.

"So he will always be Jack to us. Jack worked so hard to be a part of our little family, and he will be, forever.

"To our Jack - I’m so sorry that the first few moments of your life were met with so many complications, that we couldn’t give you the home you needed to survive. We will always love you."

Now, a month on, Teigen has unveiled a small but touching tribute to Jack - a delicate tattoo on her wrist.

Sharing a series of photos to her Instagram Stories from a date night with Legend, 41, on Saturday, one picture showed Teigen's new tattoo.

Holding hands with John, Jack's name could be seen tattooed on Teigen's wrist - just below the previous inkings of John's name, and the names of their other children, Luna and Miles.

In a blog post shared on October 27, Teigen opened up in great detail about her miscarriage.

Chrissy chose to address the reason why she shared pictures from her heartbreaking ordeal, which led to her being trolled on social media.

She explained that despite her husband's reluctance, she took them as she "needed to know of this moment forever."

She wrote: "The same way I needed to remember us kissing at the end of the aisle, the same way I needed to remember our tears of joy after Luna and Miles. And I absolutely knew I needed to share this story."

She added: "I cannot express how little I care that you hate the photos. How little I care that it's something you wouldn't have done.

"I lived it, I chose to do it, and more than anything, these photos aren't for anyone but the people who have lived this or are curious enough to wonder what something like this is like.

"These photos are only for the people who need them. The thoughts of others do not matter to me."

Chrissy's mother shared this heartbreaking video of herself cradling Jack:

The 34-year-old said that she has been experiencing a rollercoaster of emotions since her miscarriage and has been especially grateful to her children, Luna, four, and Miles, two, during this time.

"I find myself randomly crying, thinking about how happy I am to have two insanely wonderful little toddlers who fill this house with love," she wrote.

Chrissy concluded her heartbreaking essay by thanking those who have wished her and his family well.

"I wrote this because I knew for me I needed to say something before I could move on from this and return back to life, so I truly thank you for allowing me to do so. Jack will always be loved, explained to our kids as existing in the wind and trees and the butterflies they see."
