Perez Hilton was treated for a drug overdose by first responders at his Miami home after the celebrity blogger engaged in self-harm during a livestream on August 4, according to a police report obtained by People.
The report reveals that fire rescue personnel treated Hilton, whose real name is Mario Lavandeira Jr., for multiple injuries and an overdose at the scene before he was taken to hospital.
It is not clear what drugs were involved.
His sister Barbara told police she found her brother naked and 'covered in blood' in his bathroom after he had locked his bedroom door earlier that day.
According to the Miami-Dade County Sheriff's Office incident report, Barbara Lavandeira told officers her brother had been 'unusually quiet' that day and had locked his bedroom door, which she said was out of character.
She knocked and asked if he was OK through the door. He said he was fine.
When he eventually unlocked it, she saw him naked and covered in blood. Her immediate instinct was to get his three children, aged between 8 and 13, out of the house. She took them to her car.
Before they could leave, Hilton walked outside. He was still naked and bleeding. He began banging on the driver's side window. Barbara drove away once she saw police arriving at the property.
Officers and family members entered the home, brought Hilton into the living room, and fire rescue treated him there for both his injuries and an overdose before transporting him to hospital.
Hilton, 48, had an exceptionally difficult year even before August 4.
In March, he was hospitalized for 21 days with severe sepsis after a perforated ulcer.
He was on a ventilator.
He later said 'God presented himself' to him during the crisis and posted a nine-minute video publicly apologizing for years of cruel behavior as a celebrity gossip blogger.
"I made a lot of mistakes in my past," he said in the video.
"My mistakes are very public. The internet is forever. I carry with me deep shame and regret."
In April, he was back in hospital for emergency surgery after a 'massive' blood clot.
By late April, he was celebrating being able to walk again using a walker, calling it 'the most excruciating pain of my life'.
His sister told police that since the sepsis hospitalization, Hilton had been dealing with depression, anxiety, and 'overwhelming thoughts'.
Hilton's family confirmed last week that he had 'experienced significant blood loss and sustained additional injuries that will require surgery in the coming days'.
They said his recovery will be long and warned the public to be cautious about believing unauthorized reports.
His management team at Golden Artists Entertainment said in a statement: "Our focus remains on Perez's health, recovery, and the privacy of both him and his family during this incredibly difficult time.
"We respectfully ask that everyone continue to honor his privacy while he receives the care he needs."
Neither Hilton's family nor his representatives have publicly addressed the overdose detail from the police report.
Hilton remains in treatment.
He is a single father to three children. He made a name in the early 2000s as one of the internet's first celebrity gossip bloggers, known for a deliberately provocative style that drew both a massive audience and widespread criticism.
In recent months, following his near-death experience with sepsis, he had been publicly reflecting on that legacy and trying to make amends.
He had seemed, by his own account, to be turning a corner.
The events of August 4 suggest the reality was more complicated than that.
