Modern Family actress Sarah Hyland has been very open about her various health struggles, one of which saw her undergo two kidney transplants.

Diagnosed with kidney dysplasia, the 28-year-old actress had her first transplant at the age of 21. Unfortunately, her body rejected the kidney her father donated, so her brother had to donate a second.

Needless to say, it was an incredibly difficult time for the young actress, who admitted to contemplating suicide in an interview with SELF last December.

"I was very depressed," Sarah says. "When a family member gives you a second chance at life, and it fails, it almost feels like it's your fault. It's not. But it does."

Naturally, the surgeries she underwent to treat her kidney dysplasia made significant changes to her body. In particular, she had a noticeable bump around her belly - a bump that she had previously chosen to conceal using Spanx.

In fact, earlier this year, Sarah took to Instagram to declare to her millions of followers that she had been wearing - not one, but two - pairs of Spanx underneath her gown at the Vanity Fair Oscars after-party.

"I’m wearing TWO pairs of spanx," she revealed in the caption. "Why diet? When you can just hide it!"

This time around, however, Sarah bravely decided against wearing the shape-wear. Yep, her performance with Jordan McGraw at the Teen Choice Awards on Sunday night saw the star take the stage in a tight pink dress, sans Spanx.

She went into detail about her decision to refrain from concealing her bump in the touching post on Instagram:

"Yesterday I had my first nationally televised performance of a song I did with the amazing @jordanmcgraw. I was terrified. I was excited. There was a PHENOMENAL crowd supporting us the entire way through. "Thank you so much to everyone who watched! Thank you @teenchoicefox for having us!! And a final thank you to my ever changing self confidence for making the decision to not wear spanx and let my (kidney upper p***y area) shine like the badass (b**ch) she is."
