New details surrounding Matthew Perry's death have been revealed on his death certificate, following his tragic passing at the age of 54.
Fans and loved ones of the star were left shocked after news of the beloved Friends star's death broke at the end of October.
Perry had been found unresponsive in his hot tub at his home in the Pacific Palisades area of Los Angeles on October 28, and was laid to rest in a private funeral on November 3.
While the official cause of his death remains deferred awaiting the results of toxicology tests, new details have emerged on his death certificate.

According to the document, which was obtained by the New York Post, the informant is listed as Perry's stepdad, 76-year-old Dateline journalist Keith Morrison, who is the one who provided details for the death certificate.
It also states that Perry's official time of death was listed as 4:17PM after emergency services were called to the home around 4PM that afternoon after reports he was unresponsive.
Previous reports stated that Perry was found submerged in the hot tub by a bystander, believed to have been his assistant, however, the person that made the initial 911 call is yet to have been named.
The death certificate also states that Perry's occupation was an "actor" in the "entertainment" industry, in which he had worked for 38 years.
An autopsy was performed on Perry's body before it was released to his family for burial and his official cause of death has been deferred until the results of toxicology tests are returned, which can take several weeks to complete.
Initial toxicology reports also confirmed that the actor did not have any meth or fentanyl in his system at the time of his death.

Dr. Michael Baden, former chief medical examiner of New York City, told Fox News Digital on Friday that the full toxicology report may already be complete.
He explained: "They’ve got pretty much 99% of what they’re going to get at this point as far as information goes, and they can release it now, or they can wait for a couple more weeks because they’re looking for some unknown drug.
"They might be doing microscopic slides, but it’s now about almost two weeks, so the toxicology should be finished."
The beloved actor, who was best known for playing Chandler Bing in Friends, was laid to rest on November 3 in a private funeral attended by all of his castmates.
Jennifer Aniston, David Schwimmer, Courteney Cox, Matt Le Blanc, and Lisa Kudrow, who starred alongside Perry in Friends from 1994 until 2004, were all seen attending the service before he was buried in the Forest Lawn cemetery in the Hollywood Hills, across from where the NBC sitcom was filmed.
His Friends co-stars shared a statement saying: "We are all so utterly devastated by the loss of Matthew. We were more than just castmates. We are a family.
"There is so much to say, but right now we’re going to take a moment to grieve and process this unfathomable loss. In time we will say more, as and when we are able.
"For now, our thoughts and our love are with Matty’s family, his friends, and everyone who loved him around the world."
