The US Coast Guard confirmed to the Daily Mail that the incident occurred while Meadows was working aboard the Aleutian Lady during the vessel’s operations in the Bering Sea.
According to officials, Meadows was pulled from the water approximately ten minutes after he went overboard but was found unresponsive.
A representative for the US Coast Guard released a statement explaining what happened that evening.
“On February 25, 2026, at 5:05PM local Alaska time, Coast Guard watchstanders at the Arctic District command center in Juneau received notification from the fishing vessel Aleutian Lady, reporting that a crew member by the name of Todd Meadows fell overboard approximately 170 miles north of Dutch Harbor.
“He was recovered unresponsive by the crew approximately ten minutes later. First aid and attempts to resuscitate were unsuccessful, and the crew transported the deceased to Dutch Harbor.”
After the vessel returned to port, Meadows’ body was transported to Dutch Harbor and later received by the Anchorage Medical Examiner’s office in Alaska on February 27, according to a source who spoke with TMZ.
An autopsy was conducted, and his body was released on March 2.
The Coast Guard has confirmed that an investigation is now underway to determine the circumstances surrounding the fatal incident.
“The Coast Guard is currently investigating this situation. As the nation's leading marine safety organization, the Coast Guard investigates marine casualties and accidents to uncover their causes and initiate necessary corrective actions.”
Captain Rick Shelford calls it “the most tragic day” in the vessel’s history
News of Meadows’ death was first shared publicly by Captain Rick Shelford, who leads the Aleutian Lady and appears on Deadliest Catch.
Shelford announced the tragedy in a Facebook post on Monday morning, describing the devastating loss for the crew. He said Meadows died aboard the vessel on Wednesday in what he called “the most tragic day in the history” of the Aleutian Lady.
Meadows was also a father to three young boys.
In his tribute, Shelford explained that the young deckhand had only recently joined the crew but had quickly become an important part of the team.
“His smile was contagious, and the sound of his laughter coming up the wheelhouse stairs or over the deck hailer is something we will carry with us always.
“Todd's love for his children, his family, and his life was evident in everything he did. He worked hard, loved deeply, and brought joy to those around him.”
Discovery Channel releases statement following tragedy
The fishing incident reportedly occurred while filming was taking place for the reality show, although it remains unclear whether cameras captured the moment Meadows fell overboard.
According to sources cited by TMZ, a memo was reportedly sent to producers of Deadliest Catch informing them about the tragedy and providing resources to help crew members and production staff deal with grief.
A spokesperson for the Discovery Channel also released a statement expressing condolences following Meadows’ death.
“We are deeply saddened by the tragic passing of Todd Meadows.
“This is a devastating loss, and our hearts are with his loved ones, his crewmates, and the entire fishing community during this incredibly difficult time.”

“Right now, our hearts are broken in a way that words can't fully express.
“We ask that you lift Todd's children and family in prayer and keep them in your thoughts as they face the days ahead without him.”
Although Meadows had only been on the boat for a short time, the captain emphasized the lasting mark he left on the crew.
“Todd will forever be part of this boat, this crew, and this brotherhood. Though we lost him far too soon, his legacy will live on through his children and in every memory we carry of him.
“Rest easy brother, till we meet again.”

Following the tragedy, a GoFundMe campaign was launched to support Meadows’ family and help cover expenses during the difficult months ahead.
The fundraiser was organized by Paige Knutson and describes the devastating loss experienced by his loved ones.

The page explains that the funds will help the family navigate the immediate aftermath of the tragedy, including funeral costs and financial support for his children.
“This unimaginable loss has left his children without their father, his parents without their son, his siblings without their brother and his family broken.
“As the family navigates this heartbreaking time, we are raising funds to help support his three boys, assist with funeral expenses, and help ease the financial burden in the months ahead. Any donation, no matter the size, will go directly toward honoring his life and ensuring his boys are cared for in the way he would have wanted.”
