A baby tragically lost its life after its mother drank "two or three" bottles of wine and fell asleep in a chair beside it, The Sun reports.
The six-month-old, who is being named as Baby A in media reports, was tragically found by its father after he arrived home from work.
Per a report carried out by Derby and Derbyshire Children's Safeguarding Board, the little one had suffered "preventable harm".
Baby A's mother had a history of "chaotic alcohol abuse", according to the same report.
The report drew parallels between the mother's substance abuse and periods of difficulty in her life, particularly during the fertility treatment she underwent for her first pregnancy.

She had previously received treatment for depression and anxiety but this came to an end during her pregnancy.
When she was expecting Baby A, the mother underwent assessment and told a midwife she did not drink alcohol.
But later she admitted to smoking and drinking when she was pregnant, and after welcoming Baby A, she was put back on medication for anxiety and low mood.
During visits from health officials, the mother reportedly told them she was finding it difficult to take care of both Baby A and her toddler.
At one point, her sister even reported her missing with her kids and there were concerns alcohol may have been involved in the incident.

Her husband had also told police that she had driven with her kids in the car while under the influence of alcohol.
The report stated: "The outcome was an acceptance of mother’s script that her alcohol usage was under control."
No risk assessment was carried out in spite of the many signs that she had a troubled relationship with alcohol and was a potential danger to her young kids.
When Baby A's dad came home from work, he saw his little one between his wife and the arm of the chair, saying the infant appeared "lifeless".
Despite attempts to resuscitate Baby A, the youngster tragically died just two days later.
The mother allegedly confessed to drinking two or three bottles of wine and having fallen asleep holding her baby.
The report stated that Baby A's father "subsequently presented a picture of persistent, heavy drinking for many years, which was very different from that presented by Baby A’s mother herself".
The report emphasized that officials should not have accepted what the mother told them about her drinking habits and should have carried out further investigation.
