A horrifying incident saw a woman fall four storeys to her death after the doors to an elevator shaft opened without the lift being there.
Clelia Ditano, 25, was in her apartment building when she pressed the elevator button.
When the doors opened, she stepped through, unaware that the elevator was still four floors below.
According to Italian media reports, Ditano fell onto the roof of the elevator, which had stopped on the first floor, and died on impact.
Ditano's father made the heartbreaking discovery of her body after hearing her phone ring from within the elevator.
He promptly alerted emergency services, who arrived at the scene around 8:00AM.
Just hours before the accident, Ditano had posted a photo on her Instagram story, wearing an off-white top and matching skirt, seemingly ready for a night out.
In a haunting message, she had also shared an image of herself in a black dress with the caption: "Here's to what's to come."
Friends of Ditano, who worked as a cleaner for several bed and breakfasts in the area, described her as a vibrant young woman who loved dancing and socializing, per the Daily Mail.
Ilenia, a close friend, expressed her grief in a tribute: "It wasn't supposed to go this way, you were supposed to make all your dreams come true, the family you wanted to create, find true love..."
People have offered their condolences in the comments of Ditano's final post, with one person writing: "Condolences to all the family.. You can't believe it.."
A second said: "You can’t die like that, rest in peace and may God receive you. I have no other words but my thoughts are with the family."
A third added: "How strange to see the story still available and this last post both published a few hours ago.. not even 24 hours have passed since the last story and yet you're not here anymore, this thing gave me shivers, everything ends in a second. I hope you'll find peace."
Authorities, including police officers and firefighters, are investigating the incident to determine why the doors opened without the elevator present and why the lift was stuck several floors below.
An investigator told local media: "We can assume a malfunction of the locking system because the door should only open when the cabin is at the floor."
Kings III Emergency Communications, a US firm specializing in emergency services for elevators, explained that elevator systems are designed so that the car controls the opening of the doors.
"If the car is not at the landing, it will not trigger the doors to open," the company stated.
"While in particular instances, such as for maintenance reasons, the elevator doors may be opened when a cab is not present, the elevator passenger should always expect a cab to meet them when the elevator doors open. It is designed this way for safety reasons."
Our thoughts are with Ditano's loved ones during this difficult time.

A new mom has taken given birth to a whole new level - by welcoming her baby in an elevator.
Déborah Bittar, from Anápolis, Brazil, was at home with her husband last month when she suddenly went into labor. But when the couple attempted to leave for the hospital, they only made it as far as their elevator when their second child decided it was time to be welcomed into the world.
Thankfully, Bittar's husband - Dr. Danilo Almeida - is an OB/GYN who was able to help his wife with their baby's arrival.
What's more, the entire moment was captured by the building's security cameras!
Now, the couple has taken to Instagram to share the incredible moment baby Julia entered the world before they would even get out of the building.
Check out the incredible footage in the video below:The footage shows the moment Almeida realized that his daughter was not going to wait - causing him to drop his wife's hospital bags on the floor and assist.
Dad Almeida captioned the post (translated): "Childbirth is a natural, physiological, and unpredictable event, that makes it incredible! [...] Julia's birth happened very quickly and unusual way, but it's important to understand that women know how to GIVE BIRTH and babies know how to BE BORN at their exact time!"
The doctor stressed: "I am not advocating for unattended births, and I believe that safety of birth is a priority, but there are totally unpredictable situations." (And if anybody should know, it's this couple.)
Despite the unpredictable nature of their daughter's birth, the father continues: "As much as we had planned another scenario for her arrival, the atmosphere in that elevator was very peaceful and peaceful, we were witnessing the birth of our daughter!"
Déborah went on to shower Bittar with praise for how amazingly she handled the situation, and described the birth as "beautiful", "quick", and "exciting".
Per The Mirror, Bittar later revealed that she was eight centimeters dilated by the time they left their apartment.
In the footage, after baby Julia has surprised her parents, the OB/GYN can then be seen handing their newborn daughter over to Bittar while he cuts the umbilical cord. Mom Déborah can then be seen cradling her daughter in her arms.
Déborah later said that of all the births he has witnessed, this one took him by surprise.
Congratulations to the family!

A tragic incident unfolded at an LA Fitness gym in Kent, Washington, when a mother of four, Delrie Rosario, died after a fall from a treadmill.
The 36-year-old, who was accustomed to working out daily with her sister, Marissa Woods, fell and hit her head during their routine session, KIRO-7 News reported.
According to Woods, "She tried to slow the machine down. I thought maybe she just missed a step. She just collapsed, [and] hit her head on the machine."
In the ensuing chaos, Woods called for help, desperately asking for anyone who knew CPR. While other gym-goers rushed to assist, Woods noted that "not one worker" intervened, suggesting they were in shock.
Tragically, Rosario never regained consciousness after the incident and later succumbed to her injuries at a local hospital.
However, her story doesn't end there. An organ donor, Rosario ended up saving five lives, according to her sister. "Five people, literally like right now! [She’s] saving lives. How big can your heart be to still be saving lives?" Woods pondered, adding, "Just think, somebody’s walking around…with her big heart. They don’t even know what heart they’re about to get."
Known for her tireless work ethic, Rosario worked two jobs to support her children, Delaino, Rickey, Ric’Kae, and Delaiah. "[She was] a mother first. Always everything she did was for kids. She worked so hard for kids," stated Woods. In her honor, Rosario’s co-workers started a GoFundMe campaign to support her children, describing her as "an Angel walking amongst us here on earth."
Woods referred to her sister’s fall and subsequent death as a "bizarre accident." While fatalities from treadmill accidents are uncommon, injuries are not. A 2019 report from the Consumer Product Safety Commission, National Electronic Injury Surveillance System, noted that around 400,000 Americans are treated annually for exercise equipment-related injuries.
Treadmills, in particular, lead in home exercise-related injuries, causing nearly 16,000 emergency room visits in 2020 alone. Harley Pasternak, a trainer, advised People, "Anytime you go to a gym, make sure that you either have somebody else with you or you tell someone where you’re going. This doesn’t only happen with treadmills but all forms of exercise – and often there’s no one around to help."

A Texas woman had to have her hands and feet amputated when she nearly died after giving birth to her daughter.
As reported by ABC News, 29-year-old Krystina Pacheco, from Pleasanton, welcomed her baby girl Amelia on October 24, 2022, after a terrifying C-section delivery.
Two days after returning home, the mother began feeling unwell and experienced shortness of breath. She thought that the symptoms were related to her recovery so she took ibuprofen as prescribed by a nurse.
However, Pacheco continued to feel sick so she visited a doctor, who ended up dispatching her to a local emergency room. She was then airlifted to a San Antonio hospital and was diagnosed with septic shock - an incredibly scary condition in which the body has an extreme response to infection.
"I just remember I couldn't breathe anymore and I couldn't see anymore and I just started slowly fading out," Pacheco told the outlet.
"My husband, I could just hear him saying, 'Please come back to us, please, your babies need you. I need you. I need you to be here and help me with our babies,' and that's the last thing I remember," she added, as cited by People.
The mother was given life-saving treatments to prevent the infection, but tragically, it cut off the circulation of blood to her hands and feet - resulting in the necessity to lose them.
"My hands and feet were black. They looked like a person who had gotten frostbite," Pacheco said, stating that the medical team had done everything possible to avoid getting to this point.
"I was just breaking down and being absolutely crushed," she said of the necessity for amputations, per Good Morning America. "And crying with my family, crying with Jacob, and just being sad that my life would no longer be the same."
Despite it all, Pacheco held her head high and saw her husband, daughter Amelia, and son Owen as her strength to get through the multiple surgeries.
Last month, she was able to reunite with her little family and said that they are "making those adaptations and adjusting," after her painful circumstances.
Speaking to Khou 11, Pacheco stated that the whole journey has been a "roller coaster" and added: "I'm not going to say I don't have my bad days because I do. It's an emotional thing to experience."
In addition to mothering her children, she shared that she's made improvements with personal care things that most people would take for granted.
"I've gotten into putting on makeup by myself, I put my contacts in by myself. Brush my hair," she shared with the publication.

Viewers have been left "traumatized" by a chilling simulation that shows the tragic moment a woman lost her foot after it became trapped in a moving elevator door.
Back in 2017, a short-haired woman suffered a horrific accident inside an elevator at the Conch Building on Zhongshan West Road in Shanghai.
Surveillance footage, obtained by Daily Mail, captured the moment she was distracted by her phone, and stepped into the lift - without realizing it had already started moving.
As she entered, she tripped and fell forward, her right leg slipping between the closing doors. In a horrifying turn, the elevator suddenly ascended, trapping her limb and dragging it upwards.

A second woman, who had been walking behind her, avoided the same fate by hesitating at the door.
The malfunctioning lift continued to rise for three floors before stopping. The woman, still conscious, lay on the floor awaiting help - her lower leg completely severed below the knee.
The gruesome incident was recreated in a simulation by Zack D. Films, a content creator known for digital reconstructions of real-life tragedies.
"She tripped on the rising lift and fell forward. Her leg slipped between the closing doors. It continued to ascend, dragging her trapped leg with brutal force," Zack narrates in the clip shared on Instagram.
"Within moments, her leg was completely severed, leaving behind only a stump. Rescue teams arrived quickly, but the damage was catastrophic," he added.
According to the YouTuber, the elevator’s faulty operation, combined with the woman’s distraction, turned the situation deadly.
The graphic simulation has shaken social media users, with many admitting they were left traumatized after watching the clip. "I'm f***ing traumatized,"** one viewer commented.
"Traumatized in 34 seconds," another wrote.
"That’s why I’m afraid to use elevators," a third shared. "This made my heart sink into my rib cage," a fourth added.

The YouTuber shared another chilling simulation, which showed how a woman's hair was caught in a conveyor belt at a milk factory.
The horrific incident took place at an Aavin dairy plant in Tiruvallur, India, according to the New Indian Express.
The victim, 30-year-old K. Umarani, was a contract worker at Sasi Enterprises, and she was working on the production line when her dupatta (a long scarf or shawl) got trapped in a conveyor belt, which further led to her hair getting caught.
"Before she could react, she was pulled in, and her hair was caught as well. She was instantly decapitated. Other workers alerted plant authorities and police,” a police official said.
Following the accident, Milk and Dairy Development Minister T. Mano Thangaraj called it an “unfortunate accident caused due to negligence” and announced changes to factory protocols.
“According to the standard operating procedure (SOP) at Aavin plants, wearing a shawl is not allowed in the packing section. However, a few female staff members preferred to wear it, so they were permitted to do so. The SOP will be revised to make wearing an overcoat mandatory,” he added.
Thangaraj also said that the conveyor belt in the packing section moved at a slow pace and had no mechanical faults.
Police ended up arresting Varun Kumar, the supervisor of the Kakkalur dairy unit, on charges of causing death due to negligence. Additionally, Ajith Kumar, deputy general manager of the unit, was suspended.
Aavin also confirmed that departmental action had been initiated against officials from the company’s engineering department and two other staff members.
