Presley Gerber's ex-girlfriend Lexi Wood has shared a heartbreaking statement following his tragic death at 27.
The late model, who is the son of Cindy Crawford and businessman and Casamigos co-founder Rande Gerber, and older brother of Kaia Gerber, died on Sunday (September 20), at a rehab facility.
His death was first confirmed by Allie Jenkins, a representative for Crawford and Gerber, who said in a statement: "The family is asking for privacy during this very difficult and painful time."
A cause of death has not been disclosed.
A 911 dispatch audio call obtained by Page Six revealed that the 27-year-old had suffered a medical emergency, as an automated voice saying "cardiac arrest" and "overdose" could be heard.
The address provided in the call also matched the California rehab center where Presley was at the time.
An operator was heard upgrading the call, and a source at the scene said authorities, ambulance crews, and firefighters responded.
It was also reported that police were on the scene for three hours.
Presley's ex-girlfriend Lexi Wood - who starred in the reality TV show Summer House - shared a heartfelt tribute honoring the late model on Instagram.
"It’s hard to put into words what it means to lose someone who was such a meaningful part of my life for so many years," she captioned a series of images with Presley. "Through every chapter and every version of our relationship, friendship was always at the core of it.
"Everyone who loved Presley and was loved by him knows he was one of the kindest, most open, loving, and courageous people," she continued. "He loved so deeply, so purely, was incredibly romantic and thoughtful."
The 28-year-old, who dated Presley in 2022, spoke about her ex-boyfriend's strength and his openness in sharing his mental health journey.
"He fought so hard for his life, and he was brave enough to be open about that fight," she penned. "More than anything, he wanted to use his story to help other people, and he did."
"I feel so lucky to have shared so many years of friendship, so many memories and so much love with him. He will always hold such a special place in my heart, and we will all miss him forever," Woods concluded.
In the years before his passing, Presley launched a "Mental Health Mondays" series on social media to support others on a similar journey through their struggles.
"I really hope you all join me on my journey through life and pursuit of happiness," he wrote on Instagram in March 2023. "Let’s learn from each other so we can help each other."
In November 2025, after stepping back from Instagram, he returned with a message for his supporters.
"It's been a minute since I've posted," he penned at the time. "I had to step back and take care of my own mental health for a bit.
"But I'm coming back soon - stronger, clearer, more intentional than ever," he added. "When I started Mental Health Mondays, the goal was to spark real conversations about the stuff most people hide. It's incredible seeing how far that message has spread and how many people it's helped."
A month later, Presley took to the platform to speak about his dependency on prescription medications.
At the time, the model - who said he was sober at the time - said he was taking different types of medication.
"Long story short, where I'm at currently, [four] classes of medications: blood pressure, opiate, benzo, anti-depressant, Xanax - when the panic attacks are really, really bad," he said, per Entertainment Tonight.
"The Valium I take a little bit in the morning or not… I don't know if it's a little or a lot."
He continued: "Anti-depressant wise...Prazosin, also used to treat night terrors and PTSD, and ketamine drips - I definitely use that more than I should. Sometimes, it's not in a clinical setting," he added.
Presley said he wanted to wean himself off the drugs and expressed frustration over what he described as a lack of clear guidance from doctors on how to safely reduce the medications he had been prescribed.
"Once you start taking them, if you don't take them, the quote unquote withdrawal from certain medications. But I'm just looking for a doctor that's going to actually help me taper," he said. "Every psychiatrist I've had, and I've had like 15, they're all just like, 'Well, here's 20 medications.'
"I'm just trying to hopefully find a good community of people that can be good influences on me as I've grown up in Hollywood," he added.
Four weeks later, Presley returned with another update and said he was cutting down on his medications while trying to keep himself stable.
"I am cutting down. So kind of got rid of the Xanax every day by a lot [and] take a tiny little bit - maybe once a week. So that's good," he said.
He also said he had started attending meetings and was focusing more closely on his health.
"Going to meetings which can be helpful. Yeah, just focusing on my overall health. There's a lot of us out here that feel odd around the holidays because you want to be your best self in front of your family," he added.
