A urologist who has spent 40 years examining thousands of penises at one of the most prestigious medical institutions in the world has given a definitive answer on what actually counts as small.
Dr Arthur Burnett, the Patrick C. Walsh Professor of Urology at Johns Hopkins University, says it is the question men want to ask more than almost any other. They just never ask it first.
"More often, they come to see me about something else, such as erectile dysfunction or a rash," he wrote in the Daily Mail.
"Then, as the consultation draws to a close, they'll finally ask me: Doctor, is my penis too small?"
According to a 2023 global study of over 55,000 men, the average erect penis length is 5.5 inches.
The average flaccid length is 3.4 inches.
That is the global figure. It varies significantly by country.
Data from WorldData ranks Ecuador first in the world for average erect penis size at 6.93 inches. Cameroon is second at 6.56 inches. Bolivia is third at 6.5 inches.
The United States comes in 60th at 5.35 inches. The UK is 68th at 5.17 inches.
The top 20 also includes the Netherlands (6.24 inches, ranked 9th), France (6.2 inches, 11th), Canada (6.19 inches, 13th), and Italy (6.04 inches, 19th).
Medically, a penis is only considered abnormally small if it falls 2.5 standard deviations below the mean.
That translates to an erect length of roughly 3.67 inches or less, a condition formally diagnosed as micropenis.
Only about 2.28% of men fall into that category.
That means for the vast majority of men who worry their penis is too small, it isn't.
Research consistently shows that around 45% of men are dissatisfied with their size, but the overwhelming majority of those men are within the normal range and have no medical basis for concern.
Because most men's only frame of reference is pornography, which is about as useful for understanding average penis size as Fast and Furious is for understanding average driving.
Dr Burnett, who helped pioneer the science behind Viagra through his research into the nitric oxide mechanisms that control erections, has spent decades reassuring patients that what they are looking at online is not representative of the general population.
A 2025 study in Urology Research and Practice found that men who measured their penis in an erect state, rather than flaccid, reported significantly less anxiety about their size.
Part of the problem is that men judge themselves flaccid and compare themselves to others erect, which is not a useful comparison.
Less than most men think.
Some research suggests that girth matters more than length for partner satisfaction.
Other studies have found that the ideal size varies depending on the height of the man and the preferences of the individual partner.
What the research is fairly consistent on is that confidence, communication, and technique matter more than measurements.
Studies on the orgasm gap between men and women have found that the disparity has almost nothing to do with anatomy and almost everything to do with how sex is approached.
Dr Burnett's article references a 'growing' treatment.
The only non-surgical option with consistent clinical evidence behind it is penile traction therapy, which involves wearing a medical stretching device for several hours a day over a period of months.
Studies have shown modest gains in length, typically around 1-2cm, though results vary.
Surgical options exist but carry significant risks and are generally only recommended for men with a diagnosed micropenis or those who have experienced penile trauma.
